Prior to your first appointment it is important for us to get to know each other. We use an Intake Form to gather some basic information about you, including your medical and psychiatric history, current issues you may be experiencing, and a host of other questions designed to help us assess your current level of emotional, physical, and psychological wellness and functioning. Our Informed Consent/Disclosure Statement tells you a little information about us, such as education and training, theoretical orientation, rights and responsibilities as a client, and other policies related to our therapeutic relationship. All information you share with us is confidential and our HIPAA/Notice of Privacy Practices form and our Disclosure Statement overviews how your health information is protected, and certain limits surrounding confidentiality. Our Credit Card Policy form overviews our clinic policies regarding payment, as we do not accept cash or checks. Also, we require a 24 hour cancellation notice to avoid being charged the full fee for your service if you need to reschedule your appointment. We ask that you review these forms thoroughly prior to your first appointment. During your first appointment we will review this information together and answer any questions you may have.
